* Reasonable and Customary (R&C) — For Out-of-Network services, this plan will pay a Reasonable and Customary amount based on an average of charges of physicians within the network. If services are higher than this amount, you may have to pay the difference.

Using your dental benefits

Here’s how to make the most of your dental benefits:

  • Choose a provider. Each time you need dental care, you have a choice of providers. Selecting a MetLife participating dentist will ensure you receive the highest benefits from your plan. To find a provider, go to MetLife.
  • Present your plan identification information at your provider’s office. The group number is 315674 and your member number is your Social Security number.
  • If your service will exceed $500, submit for a pre-treatment estimate. You should always submit a request for a pre-treatment estimate for procedures and services your dentist believes will exceed $500 (procedures such as crowns, inlays, bridges, and periodontics).   • Check your claim status and other information at MetLife. You can review Explanation of Benefits (EOB) statements, check if claims have been paid, and more.
No ID Cards for the Dental Plans

No ID card is provided for the dental plans. When you visit your provider’s office, simply provide the dental plan group number, 315674, if your provider needs to check your benefits coverage. Your member number is your Social Security Number.
