Life Events

Life is everchanging. Your need for benefits can be too. Some life changes make you eligible to make a change to your benefits outside of Open Enrollment or make you eligible for specific types of leave benefits.

The below checklists provide you with the steps that you need to take if you have experienced specific life events.

Lyra provides support to our employees with any life event or challenges, no matter how big or small. We encourage you to access the Lyra site to access the comprehensive suite of services and resources.

New Baby (Birth or Adoption)

  • Review the Informatica Leave and Time Off policy and process. If you plan to take Parental Leave, call (866) 779-1054 or access the Unum portal online to initiate the leave process.
  • Reach out to Aetna or Kaiser for pregnancy or new parent related resources that they offer to their members
  • Add your baby to your benefits in Workday within 31 days of the date of birth of your new baby

Getting Married or Divorced

  • Change your benefits coverage within 31 days of marriage if adding your new spouse to your benefits
  • Update your name, address, and emergency contacts in Workday, if necessary
  • Update your life insurance beneficiary in Workday
  • Update your retirement plan beneficiary in the Fidelity system
  • Update your tax withholdings and direct deposit in Workday, if necessary

Death in the Family

  • Review the Bereavement Leave Policy in the Employee Handbook
  • Review Personal Leave in the Leave of Absence Policy, if necessary
  • Submit a life event in Workday to make a change to your benefits within 31 days of the death
  • Update your life insurance beneficiary and emergency contacts in Workday
  • Update your retirement plan beneficiary in the Fidelity system
  • Update your tax withholdings and direct deposit in Workday, if necessary

MOVING -- WITHIN THE US

  • Update your address in Workday
  • Submit an Address Change life event and change your benefits coverage within 31 days of your move in Workday, if necessary due to change of state
  • Request for your HR Business Partner to change your Work Location in Workday
  • Update your state tax withholding forms, if necessary

Serious Illness

  • Review sick/disability leave policies
  • Notify your manager of your expected absence
  • Call (866) 779-1054 or contact Unum online to begin processing your leave

Leave of Absence

  • Review the Leave of Absence Life@ page
  • Notify your manager and HR Benefits Team of your expected absence
  • Call (866) 779-1054 or contact Unum online to initiate your leave
